چکیده

Background/introduction MR-guided HIFU is an effective treatment for metastatic bone pain through periosteal nerve ablation,[1] and is being investigated for treating back pain through facet joint denervation.[2] For peripheral neuropathy, ultrasound-guided HIFU has been investigated preclinically as a means of achieving either an irreversible conduction block to treat severe spasticity [3], or a reversible partial conduction block to alleviate chronic pain.[4] However, ultrasound offers limited visualization of deeply situated pelvic nerves [5] and lacks the ability to measure thermal dose, which predicts the extent of changes in peripheral nerve histology and function [6]. Recently developed diffusion-prepared 3D MR neurography imaging techniques with fat suppression and nerve-selective T2-weighting [7] could improve targeting accuracy over ultrasound guidance. We present initial investigations into the use of MRI to guide HIFU ablation of peripheral nerves in a swine model. The objectives were 1) to evaluate the feasibility of identifying peripheral nerves using MR neurography on the clinical MR-HIFU system, 2) to monitor HIFU ablation of peripheral nerves using MR thermometry, and 3) to evaluate the ability to measure thermal lesions in peripheral nerves using contrast-enhanced T1-weighted images and thermal dose maps calculated from MR thermometry.

Non-Invasive Targeted Peripheral Nerve Ablation Using 3D MR Neurography and MRI-Guided High-Intensity Focused Ultrasound (MR-HIFU): Pilot Study in a Swine Model

PURPOSE Ultrasound (US)-guided high intensity focused ultrasound (HIFU) has been proposed for noninvasive treatment of neuropathic pain and has been investigated in in-vivo studies. However, ultrasound has important limitations regarding treatment guidance and temperature monitoring. Magnetic resonance (MR)-imaging guidance may overcome these limitations and MR-guided HIFU (MR-HIFU) has been us...

Early health technology assessment of magnetic resonance-guided high intensity focused ultrasound ablation for the treatment of early-stage breast cancer

BACKGROUND Magnetic resonance-guided high intensity focused ultrasound (MR-HIFU) ablation is in development for minimally invasive treatment of breast cancer. Cost-effectiveness has not been assessed yet. An early health technology assessment was performed to estimate costs of MR-HIFU ablation, compared to breast conserving treatment (BCT). METHODS An MR-HIFU treatment model using the dedicat...

Ablation of the sacroiliac joint using MR-guided high intensity focused ultrasound: a preliminary experiment in a swine model

BACKGROUND Dysfunction of the Sacroiliac Joint (SIJ) is one of the key sources of low back pain. For prolonged pain relief, some patients undergo fluoroscopic guided radio-frequency (RF) ablation of SIJ, during which a number of RF probes are inserted to create thermal lesions that disrupt the posterior sacral nerve supply. This procedure is minimally invasive, laborious, time-consuming and cos...
